Mental Illness Paper Obsessive compulsive disorder (OCD) is real illness that can be treated with medicine and therapy. When have OCD, you have recurring, upsetting thoughts (called obsessions). You repeat doing the same thing, over and over again (called compulsions) to make the thoughts go away. And, you feel like you cannot control or stop these thoughts or actions. The obsessions, or upsetting thoughts, can include things like a fear of germs, a fear of begin hurt, a fear of hurting others, and disturbing religious or sexual thoughts. The compulsions, or actions you repeat to make the thoughts go away, can be things like counting, cleanings hand washing, and checking on things. For a long time OCD was thought to be an infrequent disorder. In other words, it was believed to be rare among à ¢â‚¬Å"general population†. Now, it is estimated that over the course of a lifetime approximately 2.5 percent of individuals will develop this disorder. For children and adolescents, it is estimated that approximately the same percentage suffers from this disorder. In fact, this figure appears to be consistent with regard to American populations and worldwide populations, as well as male and female occurrences. Clinical and animal research sponsored by NIMH (National Institute of Mental Disorders) and other scientific organizations has provided information leading to both pharmacologic and behavioral treatments that can benefit the person with OCD. One patient may benefit significantly from behavior therapy, while another will benefit from pharmacotherapy. Some others may use both medication and behavior therapy. Others may begin with medication to gain control over their symptoms and then continue with behavior therapy. Nonetheless, it is evident they have love and mutual respect for one another. Unfortunately, their happiness is abruptly disrupted when Malcolm is shot by his former patient, a boy whom he could not help. He does this because he feels as though Malcolm failed him and blames him for being a freak. Following the shooting, we see Malcolm try to interact with his wife without success and her repeated failure to respond to him or acknowledge his presence in a room. She appears to ignore him and he views this lack of communication as anger and resentment that stems from marital neglect. This is shown in a scene where his wife is waiting for Malcolm at a restaurant to celebrate their anniversary and he arrives late because he is in a session with Cole- his new patient. His apology is futile and his wife leaves angry and hurt. Malcolm’s perception of the situation is inaccurate because of his inability to acknowledge that he is actually dead. Until this is revealed to Malcolm, he will continue to falsely interpret his wife’s behavior and actions. This point is proven when we see Malcolm find his wife’s bottle of antidepressants and his assumption that her depression is due to marital problems and not grief from his tragic demise. At the suggestion of Cole, Malcom expresses his love to his wife while she sleeps and in that moment his true reality is finally realized – he his dead! It is abundantly clear that Malcolm’s sessions and relationship with Cole are paramount in his ability to evolve and eventually say goodbye to his wife and have closure in their marriage. Malcolm believes he is Cole’s hired psychologist and desperately wants to help him. His motivation for doing so is fueled by his guilty conscience resulting from his failed attempt to help his former patient who ultimately shot him. Malcolm sees the similarities between the boys and wants the opportunity to get it right. At first, it is unclear if Malcolm can help Cole, but through a bedtime story everything changes. He narrates this to Cole as a bedtime story that revolves around a character named Malcolm, â€Å"Malcolm who works with children and. although he loves his job, he makes a mistake when he is unable to help a young boy. That mistake changed his life forever. He thinks about him all the time and cannot get the boy out of his mind. He then meets a new boy that reminds him of the other boy he was unable to help. This new boy is awesome and Malcolm wants to help him. If he helps this new little boy, it will be like helping the other one too†. From this bedtime story, Cole discerns that Malcolm is talking about himself and this makes Cole more open towards accepting Malcolm’s help. This breakthrough with Cole is the turning point in their relationship. Malcolm suggests that Cole help the ghosts that haunt him. This proves to be therapeutic and we begin to see spiritual growth and improved quality of life for Cole, which directly affects Malcolm. Malcolm’s guilt for failing his past patient lessens every day as he witnesses the positive changes in Cole. As a result of Malcolm’s persistence and success in helping Cole, he is redeemed and closer to the eternal piece that awaits him. When a person has unresolved conflict or lack of closure in a relationship it can inhibit his or her ability to move on to the afterlife upon their death. Initially, Malcom’s perception of his mortality is suppressed by the desire for redemption and marital peace. His extreme guilt over the inability to help his former patient and his decaying marriage hold him prisoner in a purgatory type state. Ultimately, through his gifted ability to help a disturbed child his eyes are opened and his soul is set free. Free from the guilt of failing a boy and his marital shortcomings, but filled with inner peace. This inner peace can be seen when he tells Cole he will see him tomorrow knowing tomorrow will never come and later that evening when he expresses his love and final goodbye to his wife allowing them both to move on. In the end, Malcolm’s spirit is free of guilt, redeemed, and living in eternal life. Through his character’s portrayal we learn the importance of living a moral, regret free and family filled life. In the Greek tragedy Antigone, written by Sophocles, there was a character named Kreon, the antagonist, who was the king of Thebes. Thebes was an autocratic state where Kreon had absolute power. Throughout the course of the play, Kreon abused his privilege of absolute power; and this caused him to suffer greatly, even though he was warned by a few people of his bad deeds. What Sophocles commented on absolute power was that one should not abuse it. If it was abused, he or she had to expect bad consequences. This was indicated by what happened to Kreon when he abused his power. Kreon settled a decree that prohibited anyone from burying Polyneices dead body. He was proud of his decree, and he also stated that he would be a good king by listening to what people said regarding his decisions. When the decree was broken by Antigone, Kreon sentenced her to death. This angered the gods because they wanted the dead body of Polyneices buried, and they did not want a live body that of Antigone buried in a cave. Kreon was told by Haimon to change his mind, but Kreon rejected his request and went ahead and buried Antigone alive. Teiresias warned Kreon that the gods were angry and his actions were to be blamed. Kreon rejected both Haimons request and Teiresias warning, and as a result, he suffered in the end. In the beginning of the play, Antigone and Ismene were found arguing about whether Polyneices body should be buried. Antigone wanted to bury her brothers body, but Ismene objected because she said that they should not disobey Kreon, who had absolute power and had prohibited Polyneices burial 26-80. Ismene indicated that the citizens of Thebes did not dare to go against what Kreon decreed. They all knew that if they objected to Kreon, punishment would be the result. In the play, Kreon was first found addressing the senate as to how a ruler should rule his state. He said in his long speech, I believe that he who rules in a state and fails to embrace the best mens counsels, but stays locked in silence and vague fear, is the worst man there. I have long believed so' 217-221. To impress the senate Kreon told them that he would listen to any advice they gave him because that was what a good ruler should do. However, this was not how he reacted when Kreon heard that somebody buried Polyneices body. While he was talking to the senate, a sentry came in and told Kreon that Polyneices body had been buried. somebody up and buried the corpse and went off: sprinkled dust over it and did the ceremonies youre supposed to' 310-312. Kreon got very angry and threatened to kill the sentry if he didnt find the culprit who had buried the body. Kreon thought that all of the sentries were bribed into not telling him who was the culprit 372-391. Koryphaios suggested that the gods might have had buried the body: My lord, we have been considering whether a god might not have done this' 350-351. Just like a dictator, as if he knew the actions of the gods, he declared that it was impossible for the gods to honor bury criminals 363-364. He defied what Koryphaios had said and just declared that the gods would never bury Polyneices, and he got his way. Before, Kreon had said that a good ruler like himself would listen to people, but Kreon did not do that. He went against what he had said. This showed that Kreon was very hypocritical, and he always only declared what he thought was right. Fire-tempered iron, the strongest and the toughest, thats the kind you most often see snapped and shattered' 578-580. Ironically, what Kreon said applied to himself. Kreon himself was stiff-minded about Polyneices not to be buried. Like all dictators, Kreon did not realize his stubbornness because he thought he was always right. Another case of when Kreon rejected others suggestions was in the scene with his son, Haimon. Kreon explained to Haimon the situation Antigone was in and the death penalty, and Haimon objected too it. Haimon said that he did respect Kreon a lot as a leader, but he said that in this situation, perhaps a second opinion will be valuable' 832. Haimon thought that no one is more innocent, no death more awful, no deeds more noble than hers' 841-843. Haimon kindly asked Kreon to change his mind for once and accept what others had to say and not give Antigone the death sentence. Kreon lost his mind and didnt accept what Haimon said, and Kreon claimed that only he was right. Again, he went against what he said earlier in the play about listening to other people. In between these years were the start of Chinese migration in Canada and the construction of the Canadian Pacific railway. Inhaled Salbutamol: Effects on the Body Introduction Salbutamol is a first-choice bronchodilator drug used in the acute treatment of asthma. Asthma is a chronic respiratory disease which causes recurrent reversible airway obstruction. An asthma attack is often triggered by an immediate hypersensitivity reaction that occurs in response to an inhaled allergen, causing acute bronchospasm in the lungs. Salbutamol treats the symptoms of asthma by reversing this bronchoconstriction. It acts as a ÃŽ ²-2-adrenoreceptor agonist, and when inhaled, it binds to beta-2-adrenoreceptors in the bronchial smooth muscle of the lungs, causing the muscle to relax. This leads to dilation of the bronchioles and improved airflow. Salbutamol has been shown to cause side effects such as tachycardia, muscle tremor, an increase in systolic blood pressure and a decrease in diastolic blood pressure. When salbutamol is absorbed into the bloodstream via the lungs, it can bind to ÃŽ ²-2-adrenoreceptors present on the heart and increase the heart rate. The stimulation of beta receptors also stimulates an increase in myocardial contractility which can increase pulse pressure. Salbutamol is usually administered by inhalation of aerosol, powder or nebulised solution, although it can also be administered intravenously. A metered-dose inhaler (MDI) is normally used for aerosol administration, which provides a standard dose of salbutamol. The spray from a MDI consists of rapidly moving propellant droplets, most of which impact in the mouth and throat where salbutamol absorption is poor, and only about 10% of the inhaled drug reaches the bronchioles. 2 This proportion may be further reduced by poor inhaler coordination since many patients fail ,3 to synchronise the firing of the aerosol with inhalation. With good coordination about 15% of the dose would be expected to reach the lungs.3 Using a spacer device has proved to raise deposition of salbutamol in the lungs to levels similar to or greater than those obtained from a correctly used metered dose inhaler. (Thorax 1984;39;935-941) The spacer is a large plastic container with a mouthpiece on one end and opening at other end where MDI is attached, and is most often used by young children and the elderly. The aim of this study was to determine the acute effects of inhaled salbutamol on heart rate, pulse pressure, and peak expiratory flow rate and to compare the efficacy of salbutamol delivered by metered-dose inhaler (MDI) with that of salbutamol delivered by metered-dose inhaler attached to a spacer (MDI-spacer). The efficacy of the two methods of salbutamol delivery were analysed by comparing the bronchodilator and cardiovascular effects of salbutamol in subjects taking the MDI and MDI-spacer. Materials and Methods 18 healthy subjects were studied with an age range of 18-35 (14 females and 4 males). Salbutamol (Ventolin ®) was supplied in pressurised, metered dose inhalers (MDI). The 18 subjects were divided randomly into three groups: placebo, MDI and MDI-spacer. Subjects in the placebo group used the placebo MDI inhaler which acted as the control. Subjects in the MDI group used the salbutamol MDI. To administer a standard dose of salbutamol, they exhaled fully, and then inhaled slowly through the opening just as the drug was released from the MDI, and then held their breath for 10 seconds. [manual]. Subjects in the MDI-spacer group used a salbutamol MDI with an attached spacer device. To inhale a single dose of salbutamol, the subjects attached the MDI to the end of the spacer, breathed in deeply through the mouthpiece and then held their breath for 10 seconds [Introduction to Exp Bio] .The standard dose released with each depression of the salbutamol MDI was 0.1mg. For each subject, control readings of heart rate, pulse pressure and peak expiratory flow rate (PEFR) were taken at 3 minute intervals for 12 minutes. At 12 minutes, two successive doses of the assigned inhaler were taken (2 x 0.1mg doses= 0.2mg), immediately after which the three variables were measured. This was repeated every 3 minutes until a total dose of 0.6mg salbutamol or placebo had been administered. Heart rate was measured manually using a stethoscope and stopwatch. Diastolic blood pressure (DBP) and systolic blood pressure (SBP) were measured by using a sphyngomanometer and a stethoscope. [manual]. The pulse pressure was then calculated by subtracting the SBP from the DSB. The PEFR was measured using a Wright peak flow meter. Statistical analyses were performed using the Statistical Package for the Social Sciences (SPSS), version 16.0. The data were assumed not to have a normal distribution, and the results obtained from the different groups were independent of each other. The non-paired, non-parametric Mann-Whitney-U test was therefore used to determine whether there were significant differences in any of the measured variables as a result of the salbutamol MDI, when compared to the placebo control. The test was also used to determine if there was a significant difference between the results of the MDI and MDI-spacer groups which would have been caused by the use of the spacer. For each subject, the overall change in a variable from the mean control value after a 0.6mg dose, was used to compare the results of subjects from different groups.
